Duty Log Form

This Form is provided for South Sussex Group Members and Members of other RAYNET groups who assist in South Sussex duties, to log time on duty, (including travel time to and from station or checkpoint), and total mileage covered for a duty, both planned and under emergency conditions. This information is only for insurance purposes and will be maintained securely at all times.

Please note: It may be possible to claim expenses incurred, but such claims must be submitted. in writing, together with receipts.

Any problems in completing this form should be notified to the Webmaster ASAP.
